- Release of Information Form. For authorizing release of confidential information from counseling records. Hard copy must be completed and signed by clients and either mailed or faxed to our office.
- Counseling registration forms. If you have scheduled an appointment with the Counseling Center, please call 232-2468 to receive instructions about completing paperwork online. If you do not complete this step in advance, please plan to arrive 15 minutes early for your first appointment.
