Benefit Eligibility for Adult Children up to Age 27
2010 Employee Reimbursement Account Open Enrollment:
Fringe Benefit Management Company (FBMC) acknowledges that State of Wisconsin employees may have had problems with enrollment. Any employee who encountered problems and was not able to enroll or missed the open enrollment can fax a signed and completed ERA Enrollment Form to Ami Pla at 850-514-5804 along with a written explanation of why you were not able to enroll or missed the enrollment period. Indicate plan year 2010 on the top of the form.
If you need to increase/change your 2010 ERA election, fax a signed and completed ERA Enrollment Form with the new annual election(s), along with a letter of explanation to FBMC, Attn: Ami Pla, fax number 850-514-5804. Print AMENDED in the top margin of the form. Remember to include both Benefits (Medical & Dependent Care) on the new form if you had elected both, even if you are not changing both.
Employees will receive notifcation from FMBC of the status of the enrollment or requested change.
